PRE-OP INSTRUCTIONS & LABS    
       

PREOPERATIVE HEALTH QUESTIONNAIRES:

Adult and Pediatric Health Questionnaires (also in Spanish for adults). If you can give this to your patients, it will greatly facilitate our preop calls and our admission process, avoiding delays for your surgeon. They may fax the forms to our toll free number 1-866-297-5257 or mail to us.

FORMS ARE AVAILABLE TO THE LEFT

PATIENT INSTRUCTIONS:

A nurse from SBSC will call each patient one to two days prior to surgery with preoperative instructions and a specific time to arrive for the surgery. We will provide you with a patient instruction sheet (included), but it would be very helpful if you would emphasize the following points.

Patients may not drive after surgery. Patients must arrange to be accompanied by an adult and they must have someone staying with them for the first night after surgery.

Patients must follow the instructions on their preop health questionnaire regarding eating and drinking. Patients should remain NPO after midnight, unless their surgery is scheduled after 12 noon. If scheduled after 12 noon, the patient may have CLEAR LIQUIDS only, up until 4 hours prior to their scheduled surgery time. Special instructions pertaining to Pediatric patients will be given during our preoperative phone call.

Patients are normally instructed to arrive at the center one hour prior to the scheduled time of surgery. It is important that they arrive promptly so that the preoperative preparation may be completed in time for surgery.

LAB WORK AND ECG’S:

SBSC has no blanket requirement for preoperative studies. All lab work, including ECG’s, are ordered at the attending physicians discretion based upont the patient’s medical status and the scheduled surgical procedure. So as to avoid confusion, we accept the same lab requirements as Cottage Hospital.
